Your Home Care Package Provider

For many older Australians who would prefer to receive help at home than move into an aged-care facility, HCP is one way to access affordable care services and receive help at home. HCP is also designed for those with more complex care needs that go beyond what the Commonwealth Home Support Programme can provide.

The funding that the individual receives through their Home Care Package can be spent on a wide range of nursing services, care services, social, support and well-being services, as well as on some types of equipment and home modification, to enable them to live safely and well at home.


No Setup Fee, Exit Fee, Basic Daily Fee or Review Fee. Just Ethical, Efficient and Effective Care.

Types of packages available

There are four levels of Home Care Packages

Basic Level

Supports people with basic everyday tasks

Low Level

Assistance with a range of tasks plus some nursing care


Supports people with intermediate care needs

High Level

Set up a meeting with our Care Expert. During this meeting your situation will be reviewed, your individual Care Plan is created and the necessary paperwork is completed to start care services.

We cover all mandatory government compliance, financial reconciliation, annual reviews & monthly reporting which are requirements of My Aged Care for all HCP holders.

Schedule your Services

Call My Aged Care on 1800 200 422, let them know you want to switch providers to Concept Care and ask for a referral code.

Speak with
your current

Speak with your current provider to agree on a date when your services will end.

Step 4


Call us and confirm the date when your services will end with your current provider. We will then arrange a time to meet with you to finalise your personalised support plan.

Our Services

Personal Care

Concept Care provides assistance with showering, dressing and toileting. This includes both stand-by and full assistance. Our caregivers are experts in all aspects of personal care and safe manual handling procedures.

Flexible/In-Home Respite Care

Concept Care provides you the respite services to enable you and your carer or family member to have a well-earned break. In home respite services means carers and family members can rest easy knowing that their loved one's routine remains uninterrupted and they are in an environment which caters to their needs. Respite care can be delivered either regularly or once-off depending on your needs.

Complex Needs Support

We provide assistance with hoisting, PEG feeding, continence and bowel care management including catheter care. Extensive training is provided for all services listed above. For any service listed outside of these, training can be arranged with a clinical nurse educator so we can meet your individual specific needs.

Domestic Assistance

Assistance with a wide range of domestic duties including cleaning, dusting, vacuuming, mopping, washing, ironing and other household tasks as required. As it is your home, you decide on the domestic duties which are performed and how our staff perform them.

Meal Preparation

Assistance with cooking and preparing meals. Our staff have been trained in safe food handling practices and can assist you with all your meal preparation needs. Choose your own meals and get our employees to prepare them.

Gardening Assistance

Concept Care provides expert gardening and yard work assistance such as regular maintenance visits, hedging & pruning, weeding, planting, mulching, lawn mowing & edging.

Community Access

Assistance to attend family and community events, go shopping, or while pursuing hobbies and interests or even when travelling on holiday. Assistance can also be provided when attending work, school or your place of study. We can even help you choose or design a community-based event which suits your needs.

Travel & Transport

Provide assistance to gain independence and support with travelling and transport to and from social and community participation, e.g. going out for a coffee with friends or playing sport.


Our caregivers provide healthy social interactions and activities that promote good physical and cognitive health. They also cater to an individual’s personal preference, encouraging activities and conversation which they love and enjoy. Companionship activities include:
