Home Care Package(HCP) scheme is a government-subsidised service accessible by older Australians(65 years and above) to support them living independently. Aged Care Services Eligibility:

Eligibility is based on your or your loved ones’ care needs, which is determined through a face-to-face assessment.

However, you must be:

• Over the age of 65; and
• An individual who requires coordinated services and supports to assist them with independently living in their home.
• In addition to, experiencing changes to your family care arrangements or having recently fallen or been hospitalized.

Moreover, the amount of funding you receive will depend on the level of your package. There are four levels of Home Care Packages to meet different care and support needs. Also, each HCP level receives a set amount of funding from the government that can use to pay for care and support services.
