Out of Hospital Care
At Concept Care, we provide you out of hospital care with the extra support you need so that you can focus on your recovery. We tailor the support to meet your needs and goals, whether personal care, community access, domestic assistance, attending your appointment, gardening assistance, or general maintenance.
This type of care could be funded under NDIS, My Aged Care, Worker’s Compensation, or other possible insurances. You can still access our support services as a private client if you are not covered with government assistance or insurance coverage.
How we can help you in your recovery?
Personal Care
We provide assistance with showering, dressing, and toileting, including both stand-by and full assistance. Our caregivers are experts in all aspects of personal care and safe manual handling procedures including hoists.
Domestic
We offer a wide range of domestic duties including cleaning, dusting, vacuuming, mopping, washing, ironing and other household tasks.
Meal Preparation
We assist you with preparing and cooking meals of your choice.
Gardening Assistance
We provide expert gardening and yard work assistance such as regular maintenance visits.
Community Access
We can assist you in attending family and community events, like going shopping, seeing a movie or even pursuing a hobby.
Travel & Transport
We help you gain independence by supporting you with travel and transport to and from social and community participation.
Social Support
We provide healthy social interactions and activities promoting good physical, cognitive, emotional, and mental health. Companionship activities include conversation, company, social outings, crafts, and more.
24-Hour care
You can stop worrying about your loved one returning home from the hospital safely or falling during the night and waiting hours for help to arrive. Our compassionate carers make your loved one’s life easier by providing round-the-clock personal care.
Complex Needs Support
We assist with hoisting, PEG feeding, continence and bowel care management, and catheter care. Training can be arranged with a clinical nurse for any service listed outside of these.
Medication Management
Medication plays an important role in helping individuals maintain their health. We have trained carers and nurses to assist individuals with their medication needs.
Assistance with Employment
We want to help clients who want to work, to get the job they like and keep it. We can assist you with creating a resume, looking for work, help you apply for community or volunteer work and get you work ready.
Flexible/In-Home Respite Care
In-home respite services mean carers and family members can rest easy knowing that their loved one’s routine remains uninterrupted and that they are in an environment that caters to their needs. Respite care can be delivered regularly or once-off ad-hoc basis.
