Concept Care is a registered Specialist Substitute Residential Care (SSRC) provider in NSW, supporting children and young people who require safe, structured and individualised care outside their usual home environment.
We understand that every child and young person has their own experiences, strengths, needs and goals. Our approach is centred on creating a safe and supportive environment where they feel respected, heard and valued.
Our Approach to SSRC
Concept Care brings extensive experience in supporting children and young people with disability, complex support needs and challenging circumstances.
Our SSRC services are built around the individual. We work collaboratively with the child or young person, their family or carers, DCJ, NDIS representatives, support coordinators, allied health professionals and other stakeholders involved in their care.
Our focus is on:
- safety, wellbeing and safeguarding;
- stability and consistency of support;
- trauma-informed and person-centred practice;
- maintaining routines and meaningful relationships;
- supporting independence and daily living skills;
- community participation and social inclusion;
- positive behaviour support and emotional wellbeing; and
- ensuring the voice, rights and preferences of the child or young person remain central to their support.
Supporting Children and Young People with Complex Needs
Concept Care has particular capability in supporting children and young people who have disability and complex or high-intensity support requirements.
Depending on the individual’s needs, our teams can support:
- complex and high-intensity disability needs;
- personal care and daily living activities;
- behavioural and emotional support needs;
- implementation of Behaviour Support Plans;
- medication and health-related support;
- community access and participation;
- development of daily living and independence skills;
- structured routines and transition planning; and
- coordination with clinical and allied health professionals.
Support arrangements are individually assessed and appropriately staffed according to the needs and risks of each child or young person.
Safeguarding Comes First
Children and young people receiving residential care can be particularly vulnerable. Safeguarding is therefore fundamental to how Concept Care delivers services.
Our governance and operational systems include worker screening and compliance checks, incident management, risk assessments, mandatory reporting processes, complaints management, behaviour support oversight, staff training and ongoing monitoring of service quality.
Our Support Workers are carefully selected, inducted and trained, with additional competency requirements applied where complex or high-intensity supports are required.
Working Together
Good outcomes require collaboration.
We work closely with families, carers, caseworkers, government agencies, support coordinators, behaviour support practitioners, clinicians, schools and other professionals to provide coordinated and consistent support.
Where a child or young person is also an NDIS participant, our experience as a registered NDIS provider allows us to understand and coordinate the disability support aspects of their care alongside their broader residential and safeguarding requirements.
A Home-Like Environment
Residential care should feel safe, welcoming and respectful.
We aim to create home-like environments where children and young people can maintain routines, participate in everyday activities and develop greater confidence and independence.
Our teams focus not only on meeting immediate support needs, but also on helping each young person build skills, relationships and positive experiences that contribute to their longer-term wellbeing.
SSRC Referrals
Concept Care welcomes enquiries regarding SSRC support for children and young people with disability, including those with complex or high-intensity support needs.
We assess each referral individually to ensure we have the appropriate environment, workforce, competencies and safeguards to meet the child or young person’s needs.
