At Concept Care, we’re a leading NDIS Registered Disability Service Provider based in Sydney. We specialize in Complex Care, Mental Health, and Behavioural Support. Our team is deeply committed to providing person-centred care, which means that everything we do is tailored to support individuals through life’s challenges—whether it’s managing a health condition or navigating mental health recovery.
As an NDIS provider, we offer unmatched in-home support services that are focused on helping individuals live a high-quality life. Our highly experienced Support Workers, backed by a dedicated Care Management team, are available around the clock to ensure that every need is met with professionalism and compassion. Whether it’s personal care, NDIS plan coordination, or mental health support, our goal is to empower individuals to live as independently as possible, all while ensuring they feel safe and supported.
Our Team and Services
Our team includes highly trained Support Workers, Clinicians, and Care Managers, all working together 24/7 to ensure that those in our care are supported in the best possible way. Whether it’s ongoing support or a moment of respite, we’re always there when you need us.

Our Journey So Far
This has been a landmark year for us, marked by growth and exciting achievements.. We’ve grown our reach, welcomed new clients across Greater Sydney and regional NSW, and strengthened the partnerships we have with allied health professionals. In fact, in 2024 we were honored to be featured on the Australian Financial Review’s Fast Starters List, recognizing our rapid growth and commitment to service excellence.
But more than any list or award, what matters most to us is the impact we’re having on the lives of our participants. Being nominated for this award is a reflection of the deep relationships we’ve built with you—the families who trust us, the participants who inspire us, and the dedicated team who make it all happen.
